Job Description

AHRC New York City is seeking a Data Records Coordinator. Under the direction of the Associate VP, Program Record Unit, and the Data Records Supervisor, the Data Records Coordinator is responsible for obtaining required Life Plan (LP) & LOC documents from Care Coordination Organization’s (CCO) & Partners Health Plan (PHP) that provide care coordination to people supported by AHRC NYC.  The Data Records Coordinator communicates with CCO & PHP in order to ensure that the agency receives Life Plan and LOC as required by regulations.  The Data Records Coordinator is responsible for reviewing and entering required information into AHRC NYC’s information systems.

Salary: $55,000 per year plus a comprehensive Benefit package. See Benefit information below.

ESSENTIAL RESPONSIBILITIES

  • Works with program departments to identify individuals receiving Care Management and coordinates obtaining Life Plan and related documents (including but not limit to Level of Care).
  • Communicates effectively and efficiently with external CCO agencies & PHP to obtain necessary documents and to ensure consistent compliance with agency documentation standards (e.g. signatures, effective dates, review dates, proper listing of services & goals, etc.). 
  • Perform reviews of Life Plan and related documentation for accuracy and request corrected document or issue acknowledgement where applicable, to ensure SAP alignment with LP.
  • Implements and maintains established protocols to ensure receipt of required documents.
  • Performs timely and accurate entry of all necessary information into AHRC NYC information systems and ensures ongoing data integrity of information.
  • Ensures that all program departments obtain access to Life Plan and related documentation needed to comply fully with regulatory standards, using reports and AHRC NYC electronic system.
  • Maintain CCO/PHP Care Manager (CM), CM Supervisor, CM Director (where applicable) and Regional Director contacts in AHRC NYC electronic systems.
  • Performs other additional duties as needed.

Qualifications

REQUIRED QUALIFICATIONS

  • Bachelor’s degree in related field (Human Services, Social Work) required.
  • Experience working for a social service provider.
  • Strong computer skills, including experience with and knowledge of Microsoft Word, Excel,  Outlook, and electronic medical or health record system,  as well as ability to conduct network scanning and file management tasks.   
  • Excellent interpersonal and communication skills required. 
  • Must have strong writing and proofreading skills and strong attention to detail.
  • Must exhibit excellent organizational skills and ability to manage multiple tasks simultaneously.    
  • Must have the ability to maintain confidentiality and discretion.
  • Demonstrable passion for social services support.
