Job Description

Summary

Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on plan of care planning decisions and coordination for 24 hour care delivery.

 

Essential Functions

  • Performs in-person comprehensive health histories and physical exams on new Albright LIFE participants. Coordinates with Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in-person reassessment semi-annually and as needed.
  • Integrates the primary care treatment plan into the overall plan of care developed by the Interdisciplinary Team. Interacts with team members to meet emergent and acute need of participants. Participates in discharge planning for acute and long-term placement.
  • Evaluates and treats participants during acute illness. Following consultation with the Primary Care Physician, refers participants to medical specialist as indicated.
  • As requested and directed by the Primary Care Physician, manages care of participants in the nursing home: Provides regular visits as dictated by nursing home standards and participant need. Performs telephone contact with nursing home staff as required.
  • Maintains participants’ medical record and fulfills Albright Life charting and reporting requirements.
  • Provides relevant participant and caregiver education. Educates and trains participant and caregivers in appropriate care and interventions. Provides staff in-service on a variety of relevant topics.
  • Completion and documentation of appropriate diagnostic coding. Orders, interprets, and follows up on lab tests and results.
  • Performs selected procedures and documents orally and in writing. Initiates emergency measures as indicated.
  • Recognizes abnormalities that justify immediate referral for intervention by others.
  • Initiates, evaluates, reviews, and changes orders for selected medications and diagnostic studies/treatments within attending physician-approved protocols.
  • Assesses, initiates, and coordinates participant care on a daily basis under the supervision of the Primary Care Physician. Request consultations from all services as needed in collaboration with Primary Care Physician and/or established protocols.
  • Acts as liaison with physicians, nurses, and other health care professionals within the program, providing education, communication, and support for Albright LIFE staff, patients, and families.
  • Directs patient management in conjunction with physician staff. Conducts periodic assessments.
  • Advises the Medical Director and Primary Care Physician in ways and means to establish better accountability of Albright Life services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.
  • Assists with the development of standards of care; performs on-going monitoring and evaluation of participant care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.

Qualifications

Experience:

  • A minimum of one year’s experience in working with the frail or elderly population required. Two years’ experience as Physician Assistant in a geriatric setting desirable. Shall have either training or related experience in the job assigned.
  • Specific business experience with older adults or in the long term care and/or retirement housing industry preferred.

Education:

  • Bachelor’s Degree from an accredited university. Graduate of certified or accredited Physician Assistant Program with five to seven years related experience with three years’ experience as Physician Assistant or a Master’s Degree and five years related experience.

Licensure/Certification:

  • Current Cardio Pulmonary Resuscitation (CPR) certification required.
  • Current Physician Assistant license in Pennsylvania.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license. Must be able to meet Albright LIFE’s auto insurance carrier's standards for coverage.
