At Home Hospice Licensed Practical Nurse (Queens/Nassau County)

Requirements

The Hospice Licensed Practical Nurse assists the Hospice Registered Nurse to provide skilled nursing care and assist in implementing the Plan of Care in the patient's residence. Nursing care involves implementation of nursing interventions and medical treatments (as ordered by the physician) and directed by the Hospice Registered Nurse as included in Hospice Plan of Care.

Location : Queens and Nassau County

Hours: 9am - 5pm

Salary: $70,904 to $77,204 annually

Qualifications: 

  • Licensed and currently registered as a Practical Nurse in New York State.
  • Adherence to safety and infection control procedures and universal precautions minimize hazards.             
  • Two years of medical/surgical experience required; Geriatric care experience useful.  Homecare/Hospice experience preferred.
  • Current NYS Driver’s License preferred.     
  • Interpersonal skills necessary to interact effectively with patients, families, and Department Staff.  
  • Analytical skills to interpret patient care data and report new data or changes promptly to the supervisor.             
  • Ability to drive to patient’s homes; ability to climb stairs, to stand, stoop, and kneel and walk safely over uneven potentially slippery surfaces for up to 20% of work day when entering/leaving and when in patient’s home.           
  • Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.

Education: 

  • LPN Degree/Diploma from an accredited school is Preferred.

COVID-19 vaccination is strongly encouraged for all new hires prior to beginning employment at Calvary Hospital.
