For over 125 years, Calvary Hospital and Calvary @ Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care. The hospice receives patients from referring institutions throughout New York City and the entire tristate area. When you come to Calvary Hospital, you're not just getting a job, you're joining a family.

The Palliative Care Social Worker provides medical social services, psychosocial and bereavement counseling to the hospice Patients and families.

Conduct a program of in-person and telephonic visits to patients enrolled in the Palliative Care Program—delivered at home, in Assisted Living Facilities (ALFs), and Skilled Nursing Facilities (SNFs)—based on individually assessed patient needs. Maintain accurate and timely documentation of visit contents, interventions, and the plan for the next visit or follow-up, ensuring continuity and quality of care across all settings.

Participate in IDT and other patient care meetings; provide recommendations to team regarding individual patient care needs, services and overall status.  Make recommendations for and facilitate transitions to other health care programs as deemed appropriate by the care team.

Works within legal, ethical practice standards to his/her position as well as Calvary’s Code of Conduct.  Complies with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer.

Qualifications: 

  1. Current licensure as LCSW
  2. At least 5 years’ experience as a hospice social worker
  3. At least 1 year supervisory experience with social workers/mental health counselors
  4. Knowledgeable in areas related to direct social work services (ie., Medicaid application, community services, etc.)
  5. Excellent written and verbal skills
  6. Demonstrated ability to work as a member of an Interdisciplinary Team on behalf of patients/families
  7. Demonstrated ability to work with peers and provide feedback and guidance in an effective manner
  8. Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.

Salary: $92,228.53 - $99,742.70

COVID-19 vaccination is strongly encouraged for all new hires prior to beginning employment at Calvary Hospital.
