Temporary Care Coordinator - ECM

ABOUT THIS TEMP POSITION

This is a temporary position and the length of assignment is estimated to go from April 2025 to December 2025. The length of the assignment is always dependent on business need and dates may change. While the assignment would be at the Alliance, if selected, you would be an employee of a temporary employment agency that we would connect you with.

WHAT YOU'LL BE RESPONSIBLE FOR

Reporting to the Enhanced Care Management Manager, this position will assist the Enhanced Health Services (EHS) team with providing health care coordination activities for Alliance members and establish and maintain effective working relationships with provider offices, County departments, community-based organizations, and internal stakeholders related to care coordination for members.

• Knowledge of:
  • Principles and practices of customer service
  • Principles and practices of managed health care and health care systems
  • Proficiency with Windows-based PC systems and Microsoft Word and Outlook
  • The diverse needs of the Medi-Cal population

• Ability to:
  • Plan, organize, and prioritize tasks, coordinate activities, and adhere to timelines
  • Listen well and ask follow-up questions to gather complete information in order to problem-solve a variety of issue
  • Effectively conduct telephone interviews in a confidential and sensitive manner and respond to sensitive or difficult issues with tact and diplomacy
  • Establish and maintain effective working relationships with members, co-workers, providers, and individuals of varying socio-economic and/or cultural backgrounds, and with special needs population
  • Identify, maintain, and protect Personal Health Information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and follow Alliance policies and procedures to ensure the security of such information

• Education and Experience:
  • High school diploma or equivalent and three years of experience related to care coordination in a health care setting which involved interacting with members, patients, and providers in meeting their service needs (an Associate's degree in Health, Social Services, or a related field or current certification as a Medical Assistant may substitute for one year of the required experience); or a combination of education and relevant work experience may be qualifying

OTHER INFORMATION

• We are in a hybrid work environment, and we anticipate that the interview process will take place remotely via Microsoft Teams.
• While some staff may work full telecommuting schedules, attendance at quarterly company-wide events or department meetings will be expected.
• In-office or in-community presence may be required for some positions and is dependent on business need. Details about this can be reviewed during the interview process.

The actual compensation for this role will be determined by our compensation philosophy, analysis of the selected candidate's qualifications (direct or transferrable experience related to the position, education or training), as well as other factors (internal equity, market factors, and geographic location).
