Your Mission: 

The Adult Day Service (ADS) Activity Worker I plays a crucial role in designing and implementing engaging activities tailored to the needs and strengths of clients at the Adult Day Service center. Reporting to the ADS Activity Supervisor, they are responsible for planning, executing, and evaluating a variety of activities that contribute to the clients' physical, mental, emotional, and social well-being. Additionally, they provide personal care and support, document client progress, and ensure compliance with program standards. The ADS Activity Worker I actively participates in developing individualized care plans and contributes to the overall quality of care and client experience.

Your Impact: 

As an Adult Day Service Activity Worker, you will, 

 

Key Responsibilities: 


Activity Programming and Implementation:
  • Plan, execute, and evaluate the ADS activity program, including suggesting improvements to the monthly and biannual activity plans. 
  • Design and implement activities as assigned by the ADS Coordinator and/or Activity Supervisor that align with the clients' physical, mental, emotional, and social strengths, preparing the room, equipment, and materials as needed. 
  • Supervise clients, assist, and encourage their participation in activities, focusing on their physical, mental, emotional, and social strengths. 
  • Lead small group and individual activities, as well as outings such as shopping trips and field trips, fostering client engagement and participation. 
  • Provide feedback to the Activity Supervisor regarding the effectiveness and client engagement in activities. 
  • Follow and implement ADS Policy and Procedures and Agency Policy on providing services to clients in the ADS center. 


Client Care and Support:
  • Understand and implement each client's individualized plan of care, reporting any changes in physical, psycho-social, and cognitive status to the Activity Supervisor, Coordinator, or Nurse. 
  • Assist with personal care services, including toileting, bathing, and meal assistance, ensuring clients' comfort and dignity. 
  • In conjunction with the Activity Supervisor, assist in Activity Assessments within one month of a new client's attendance and subsequent assessments at intervals no longer than six months, contributing to the client's care plan meeting. 
  • Implement the Healthy Identifying Depression & Empowering Activities for Seniors (IDEAS) Project to alleviate depressive symptoms through activity planning and behavioral activation. 


Documentation and Compliance:
  • Maintain accurate daily records of clients' activity participation, meal consumption, meal temperature, and workshop attendance, alerting the Coordinator and Activity Supervisor to any irregularities. 
  • Document and report incidents or matters of significant importance that occur at the ADS center or during field trips, ensuring timely and effective communication to the ADS Coordinator and Activity Supervisor. 
  • Use internal systems, including Salesforce, to document client efforts and maintain records as required. 


Collaboration and Team Participation:
  • Engage in ADS team meetings, activity meetings, client care plan meetings, and staff training sessions, contributing to a collaborative approach to client care. 
  • Work closely with the Activity Supervisor and other staff to assess client needs and provide personalized support and activities. 


Additional Responsibilities:
  • Escort drivers to transport ADS participants to and from the center, ensuring client safety during transit. 
  • Participate in other ADS-related events and activities as needed, demonstrating flexibility and a commitment to the program's success. 


Must Haves - To thrive in this role, you should have:

Education & Experience: 
  • High school diploma or GED equivalent is required with at least 2 years of working experience in a related field. 

Skills & Knowledge: 
  • Proficiency in spoken and written Chinese (Cantonese, Toishanese, or Mandarin) required; English language ability is a plus. 
  • Basic computer proficiency, including Microsoft 365 (Word, Excel) or a willingness to learn 
  • Basic understanding of Salesforce or similar data management systems is a plus or a willingness to learn 
  • Obtain and maintain a Food Handler Certificate within the first three months of employment, contributing to program safety and compliance. 


Physical Demands:
  • Ability to frequently sit, stand, and walk for extended periods. 
  • Capable of supporting or assisting clients and objects weighing in excess of 75 pounds. 


Environment and Scheduling:
  • Work is performed during standard business hours, Monday through Friday, 9:00 am – 5:00 pm, with flexibility to work additional hours, as necessary. 
  • Work is primarily conducted at the ADS center, with some outdoor activities.
