Outreach Services Manager

Wage: 65k / Annual + DOE + DOQ

Shift: Monday - Friday with on call hours

Qualifications:
  • Four-year degree in relevant field required. An equivalent of a high school diploma or GED + 2  years of experience can be substituted.
  • 3+ years of case management or social services experience; and/or equivalent combination of education and experience preferred but not required.
  • 3+ years of supervision is preferred. 
  • Knowledge of Salem service delivery system and community partners preferred.
  • Knowledge of and experience in using trauma-informed care principles required.
  • Demonstrated experience in mediation between persons exhibiting escalated behavior required.
  • Must have valid Oregon Drivers license; must have been a licensed driver for at least 3 years; must have a driving record that permits coverage under C@P’s company insurance.


Essential Job Duties Include (But Not Limited To):

Supervision
  • Provide responsive supervision to the Outreach Case Managers and Health Systems Navigator
  • Provide one-on-one coaching, facilitating restorative regular meetings, providing direction and accountability, following up on tasks, providing training, etc.
  • Attend required training, team meetings, and learn policies and procedures for C@P Operations.
  • Deliver effective development, coaching, and targeted training to empower and enhance the skills of staff team members, fostering professional growth and optimizing team performance.

Outreach Management:
  • Ensure the intake process is being followed and completed as well as provide coverage for Case Managers and/or Health Systems Navigator.
  • Coordinating outreach participant referrals. This includes all referrals into the shelter programs.
  • Conduct regular case load review with Case Managers and Health Systems Navigator.
  • Ensure appropriate representation at the Collaborative Outreach and Chronic Case Conferencing Committee and other community meetings as deemed necessary.

Maintain training and oversight of data systems
  • Oversee HMIS data entry and review data quality weekly to ensure on target with program / funder goals.
  • Maintain up-to-date knowledge of compliance requirements for each program and ensure all programs consistently meet these standards.

Staff Training Resources and Development:
  • Provide training and support of staff in their use of electronic platforms (HMIS & Activate Care) for data entry and to assure data quality.
  • Assist staff in signing up for OHP Assister Training, Traditional Health Worker related classes and training, Coordinated Entry Assessment training, and other training as deemed necessary.
  • Maintain training log for the team and ensure all members of the team are up to date on relevant training.
  • Attending leadership meetings with Church @ the Park and community partners. Practicing and upholding Church @ the Park values as well as building relationship with staff and guests.  Including team such as but not limited to Peer Support, Chaplain, Case Management and problem solving and decision making.


$65,000 - $65,000 a year

Work environment:

● Close quarters with vulnerable people including individuals without stable living situations who may be struggling with mental health and substance use concerns.

● Indoor/Outdoor work environment with frequent interruptions and demands, and occasional unpredictable behavior.

● Must be able to push, pull, and/or lift 50 pounds or less.

● Must be able to go up and down stairs.


Transportation Policy:


To receive and maintain authorization to drive a Non Profit vehicle, a driver must:  


Drivers must have a valid driver’s license of the state they reside in. Employees who are hired with an out of state license will be given 30 days (subject to DMV availability) to acquire a license for the state in which they reside.

Drivers should have at least (2) years driving experience and be at least 18 years old.  


In the past three by (3) years, drivers should have no more than: 


Two (2) At-Fault Accidents, (proof of not-at-fault status for accidents must be received to rescind this. Your personal auto insurer or DMV can assist with this information.

Three (3) Minor Moving Violations, (personal and/or business) 

Four (4) Non-Moving Violations, such as: Failure to Appear (FTA), License not in possession, or No Proof of Insurance/Registration. 

Four (4) or more in combination of the above a, b, and c.  


In the past four (4) years, drivers should have zero (0) Major Moving Violations such as: 


Driving while Suspended/Revoked and/or Invalid License Exhibition of Speed - Speed Contest,

Reckless Driving,

Driving Under the Influence (DUI) 

Vehicular Manslaughter,

Leaving the Scene of an Accident (Hit and Run) 

Speeding in excess of 100 mph, etc.
