Division/Program Summary:
The New York City Department of Health and Mental Hygiene (DOHMH) is expanding home visiting in NYC and streamlining how providers, organizations, and community residents can access quality home-visiting services. The goal is to improve maternal and child health and well-being by matching families’ assessed risks to evidence-based (EB) or evidence-generating (EG) home visiting (HV) models that are most appropriate for them based on their needs, and to connect them to other resources as needed. Racial, ethnic, and socioeconomic inequities create disparate outcomes in certain neighborhoods as compared with the rest of the city, and providing services beginning in the prenatal through postpartum period is vital to improving these outcomes. In addition, recent attention to inequities in maternal morbidity and mortality, especially in New York City, points to the need for more respectful maternity care and greater agency for people during childbirth, both of which the program’s staff will facilitate, in partnership with NYC doula organizations.

DOHMH’s New Family Home Visits (NFHV) Initiative will serve families from pregnancy through the postpartum period. A subset of families will also be offered labor and delivery support by a certified doula. NFHV incorporates a strong mental-health and chronic-disease focus, including screening for diabetes, hypertension, maternal depression, and anxiety, along with referrals to needed services. The citywide program will begin in Brooklyn, offering home visits to more than 2,900 first-time expectant parents. of those. The neighborhoods burdened by adverse maternal and child health outcomes will be prioritized; however, all first-time expectant parents are eligible.

Position Summary:
Under the Initiative Manager's supervision, the candidate will oversee NHVP site operations, supervise staff, and ensure high-quality services in TRIE neighborhoods citywide. Responsibilities include resolving database issues, conducting quality assurance, refining protocols, strategic planning, staff development, and fostering community partnerships. Additional duties include preparing reports, ensuring compliance, representing the program, and traveling across boroughs to maintain consistency and quality.

Job Duties and Responsibilities:
- Oversee program site operations and supervise home visiting staff, ensuring the delivery of high-quality services.
- Address and resolve challenges related to the Penelope Database, providing support, guidance, and ongoing training to ambassadors, home visitors, and team leaders.
- Engage in strategic program planning, development, and expansion to support NHVP (Newborn Home Visiting Program) services in Taskforce Racial Inclusion and Equity (TRIE) neighborhoods across all five boroughs.
- Collaborate with Initiative Managers and other leaders to develop evidence-based practices and refine protocols for home visiting services.
- Conduct quality assurance (QA) client visits and perform case reviews with staff to ensure complete and appropriate case management.
- Monitor and review programmatic reports to ensure program outcomes are accurately captured and communicated.
- Work closely with team leaders to ensure the consistency and quality of services provided in all TRIE neighborhoods.
- Establish and maintain strong relationships with community-based partners to enhance program impact.
- Liaise with clinical staff to strengthen referral processes, timelines, and documentation for home visitors.
- Represent the program in stakeholder engagements in the absence of program leadership.
- Support staff development initiatives to align with program standards and enhance workforce capacity.
- Collaborate with program leadership to devise strategic approaches for quality improvement and staff development to meet programmatic benchmarks
- Ensure accurate case documentation and facilitate the delivery of essential health messages to parenting families served by the program.
- Prepare and maintain administrative files in compliance with program requirements.
- Develop, review, and submit reports on program activities to enhance communication between the program and internal/external stakeholders.
- Work with leadership and staff on special projects to enhance program visibility and strengthen partnerships with new and existing stakeholders, including hospitals.
- Travel throughout the five boroughs as needed to support program operations and maintain quality assurance.

Qualifications

1. A baccalaureate degree from an accredited college or university, and two years of full-time satisfactory experience in a public health program, performing duties involving case finding, case management, interviewing, investigating and other related public health work, one year of which must have been in a supervisory capacity; or

2. An associate degree from an accredited college or university, including or supplemented by twelve semester credits in health education, or in health, social or biological sciences; and four years of experience as described in "1" above, one year of which must have been in a supervisory capacity; or

3. A four-year high school diploma or its educational equivalent approved by a State's Department of Education or a recognized accrediting organization, and six years of experience as described in "1" above, one year of which must have been in a supervisory capacity; or

4. A satisfactory combination of education and/or experience equivalent to "1", "2" or "3" above. Undergraduate college credit may be substituted for experience on the basis of 30 semester credits from an accredited college for one year of full-time experience. Twelve credits in the health, social or biological sciences may be substituted for an additional six
months of experience. However, all candidates must have a four-year high school diploma or its educational equivalent approved by a State's Department of Education or a recognized accrediting organization, and a minimum of two years of experience, one year of which must have been in a supervisory capacity, as described in "1" above.
