The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.
The RN Care Manager is an integral part of Community Health services. The nurse is responsible for creating and maintaining an active care management process for patients living in the non-congregant shelter, otherwise known as The Aspen, at 4040 Quebec. St. The nurse works collaboratively with an interdisciplinary team to support individuals in their transition to a stable housing environment. The nurse provides services in the community within the framework of professional nursing ethics and state and city regulatory guidelines. This is a field-based position and requires at least 80% of services to be provided to consumers in their transitional shelter, temporary or permanent clinic space and other community locations. The nurse provides direct nursing services to consumers of Community Health
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions - Provides skilled nursing care according to accepted community standards of quality healthcare and within program scope.
- Provides holistic, patient-centered care within an integrated, trauma-informed, culturally aware, harm-reduction model.
- Develops rapport, trust, and professional relationships with people experiencing homelessness.
- Utilizes motivational interviewing techniques to provide optimum client-centered engagement in care and continuity of services while respecting the autonomy of patients.
- Collaborates with members of the multi-disciplinary health care team and coordinates care with internal programs.
- Works collaboratively, advocates respectfully, communicates effectively, and develops partnerships with community organizations and key stakeholders.
- Maintains a work environment that is marked by respect for others; that values inclusiveness and builds workforce diversity; and that fosters cooperation and teamwork.
- Performs thorough chart reviews and comprehensive health assessments via telephone or in-person, with appropriate recommendations of care as outlined in CCH nursing protocols.
- Empowers patients by promoting health literacy and self-efficacy, providing patient education that is tailored to both the individual’s learning style, education and/or reading level, and is sensitive to their health beliefs.
- Identifies patient centered goals in which client is not passive but partner in care and develops care plan and next steps to share with team.
- Identifies social determinants of health and barriers to care. Provides advocacy and mobilizes resources.
- Provides warm hand off to assigned team members and coordinates primary care engagement for optimal health outcomes.
- Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment.
- Manages pharmaceutical and medical supplies and coordinates medication refills, makes home visits and delivers medications, administers intramuscular medications, and monitors client’s vital signs.
- Provides necessary bridge nursing services, as bandwidth allows, until patients in need can receive HCBS.
- Assists with transitional, chronic, complex care management and care coordination of health care for individuals in the community, including linkage to higher levels of care when needed.
- For patients unable to live independently / needing a higher level of care, RN will coordinate hand off to primary care team for continued healthcare services and transition to LTC.
- Participates in team meetings, in-services, supervisory sessions, and team-based or interdisciplinary quality improvement initiatives as required.
- Demonstrates competency in agency databases including HMIS and EHR; maintain accurate and timely clinical records consistent with CCH standards.
- Preceptors and train new hires and nursing students.
- Must be accessible by cell phone during business hours.
- Willing and able to transport patients in personal or CCH Fleet vehicle.
Qualifications Summary - Graduation from an accredited school of nursing.
- Licensed as ADN, BSN, or MSN in the State of Colorado.
- Certification in Basic Life Support/CPR.
- Successful completion of the privileging and credentialing process for RNs at the Coalition.
- Preferred experience in ambulatory, public, and community health nursing.
- Experience with mental health, SUD, complex care coordination, case management are beneficial.
- Bilingual English / Spanish preferred.
$31.19 - $37.42 an hour
WHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVENT YEARS OF EXPERIENCE
Qualified candidates are eligible for the NURSE Corps Loan Repayment Program which offers payment of 60% of unpaid nursing education debt in exchange for a two-year commitment at a NURSE Corps approved site with an option to extend to a third year for an additional 25% of the original balance.
