By integrating in-person and virtual primary care with urgent care, we aim to deliver personalized, hands-on care that proactively educates, connects patients with resources, and simplifies the complexities of downstream care. Your Impact 

  • Patient Care & Triage: Conduct comprehensive patient triage by taking vital signs, documenting medical history, and performing observational examinations to identify immediate healthcare needs. 
    • Assist with patient exams, ensuring all necessary equipment and documentation are prepared, while supporting providers during testing and procedures. 
  • Clinical Procedures: 
    • Perform diagnostic and testing procedures, including vision, hearing, drug screens, breath alcohol testing, spirometry, and more. 
    • Execute point-of-care testing such as strep, flu, RSV, SARS-CoV-2, glucose, lipid panel, and urinalysis. 
    • Provide cardiac and airway management, including ECGs, AED use, nebulizer treatments, and oxygen administration. 
    • Deliver wound care assessments, immunizations, and medication administration per federal and state regulations. 
  • Patient Engagement & Documentation: Assist with patient inquiries, communicate test results, and explain medication or therapy plans. 
    • Maintain accurate electronic medical records, documenting patient histories and treatment details. 
    • Observe and report changes in patients' conditions, taking necessary action as needed. 
  • Clinic Operations: 
    • Ensure medical instruments and equipment are clean, sterilized, and maintained per organizational policies. 
    • Maintain a stocked and sterile workspace, supporting smooth clinic operations. 
  • Collaboration with Clinical Team: Work closely with other members of the clinical team to ensure seamless coordination of patient care.  
    • Communicate effectively and efficiently with providers and other healthcare professionals to relay patient information and updates. 
    • Welcome and support the training of new team clinic team members.

 Required Availability 

  • Licensed Practical Nurses must have full-time availability during clinic operating hours, Monday through Friday, from 7:30am to 5:00pm. To ensure seamless patient care and clinic readiness, team members can generally expect to work from 7:15am to 5:15pm, including time for opening and closing procedures. While the schedule primarily aligns with these hours, flexibility may occasionally be required to meet patient and team needs.  

Who You Are 

  • Education: High school diploma or GED required.  
  • Certification or demonstrated prior experience: State licensed practical nurse (LPN) with prior experience placing and monitoring IVs. For Maine and New Hampshire only, must be IV certified. 1-2 years of patient triage experience in a primary care setting preferred. 
  • Basic Life Support (BLS) certification: American Heart Association or American Red Cross BLS certification. We do not accept CPR, ACLS certifications, or online only classes.  
  • Compassionate, authentic, and patient focused: Supports patients through kindness; demonstrating understanding for others and contributing to the creating a community of belonging.  
  • Proactive and confident communicator (written and verbal): Foreshadows and elevates concerns as they arise, unafraid of professional directness both with colleagues and patients.   
  • Trustworthy: Ability to maintain confidentiality and handle all patient information in accordance with HIPAA regulations.  
  • Effective communicator and cross collaborator: Ability to establish and maintain positive relationships with patients, team members, and stakeholders across the organization.  
  • Excellent team player: Enjoys collaborating with others and being a part of a strong team dynamic.  
  • Technically savvy: Proficient with Microsoft Office Suite or related software, experience with or ability to learn NextGen as well as other business systems.
