Company Description

Our goal is to place individuals and families in permanent, stable housing and provide the needed tools and support to develop self-sufficiency. Equitable Social Solutions impacts lives by creating tailored solutions for each person we serve in order to break the cycles of generational poverty and chronic homelessness.

Job Description

  • Works with participants to overcome barriers that affect their ability to obtain and retain housing
  • Manages a revolving caseload
  • Assesses clients to determine appropriate levels of service on a case-by-case basis
  • Refers clients to appropriate agencies for specialized services and counseling
  • Develops action and stabilization plans with clients
  • Documents expenditures, payments, and service issuances to clients
  • Works closely with partners and the community to identify opportunities for homeless individuals and families through outreach and education
  • Completes assessments to help participants access all available income and determine needed supportive services
  • Maintains complete, accurate, and timely case notes and related records in manual and electronic systems
  • Maintains completion of plans according to contract deadlines
  • Utilizes county and company computer data processing systems to efficiently track and record participant activity

Qualifications

  • Bachelor’s degree in Sociology, Social Work, or related field
  • Two years of equivalent work experience
  • Knowledge of local housing market and applicable regulations preferred
  • Proficient in Microsoft Office, databases, and other specialized software
  • Strong interpersonal and communications skills
