Physical Therapist (PT) – Skilled Nursing Facility | Texas

Salary Range: $95,000 – $120,000 per year

Location: Frisco, Texas

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM – 5 PM

We are a trusted provider of rehabilitation services in skilled nursing facilities throughout Texas. Our mission is to enhance the quality of life for our patients by delivering compassionate, evidence-based physical therapy within a supportive interdisciplinary environment. We believe in clinical excellence, professional growth, and putting patients first.

We are seeking a dedicated Physical Therapist to provide rehabilitative care to residents in our skilled nursing facility. You’ll work with a diverse geriatric population recovering from surgery, illness, or injury, helping them regain independence and improve mobility through personalized therapy plans.

Key Responsibilities:

  • Perform evaluations and create treatment plans tailored to patient goals
  • Deliver therapeutic interventions to promote mobility, strength, and function
  • Monitor patient progress and update plans of care as needed
  • Collaborate with a multidisciplinary team including OT, SLP, nursing, and physicians
  • Maintain accurate and timely documentation in accordance with SNF and state/federal guidelines
  • Provide family and caregiver education for safe discharge and long-term success
  • Doctor of Physical Therapy (DPT) or equivalent from an accredited program
  • Current and active Physical Therapy license in Texas
  • Experience in SNF or geriatrics preferred, but new grads welcome
  • Knowledge of Medicare/PDPM documentation requirements is a plus
