Position Overview
We are currently seeking a qualified Licensed Master Social Worker (LMSW) for the role of Nursing Home Surveyor/Complaint Investigator in New York, NY. This essential role focuses on ensuring quality of care and regulatory compliance in long-term care facilities through surveys, complaint investigations, and surveillance activities.
Key Responsibilities
- Conduct surveys and complaint investigations in long-term care facilities, including cases involving abuse or neglect
- Evaluate nursing homes for compliance with Federal and State regulations
- Draft Statements of Deficiencies (SOD) in a timely manner using the Principles of Documentation
- Assess the effectiveness of Plans of Correction and ensure timely follow-up
- Enter findings into state and federal compliance databases
- Participate in Immediate Jeopardy monitoring and state interventions when needed
- Attend relevant meetings and testify in administrative hearings as necessary
- Follow all Department of Health (DOH) schedules, policies, and procedural guidelines
- Travel to assigned facilities according to work schedule

Qualifications
Education & Licensure:
- Active New York State Licensed Master Social Worker (LMSW) license required
Experience:
- Minimum 2 years of clinical experience in a medical facility (nursing home experience preferred)
- 2 years of experience conducting nursing home surveillance or compliance activities (preferred)
- SMQT Certification preferred (must be willing to obtain within 12 months of employment)
Skills:
- Excellent investigative, documentation, and communication skills
- Strong understanding of long-term care regulations and patient advocacy
- Proficient in completing documentation, data entry, and report writing
- Willingness to travel to on-site facilities as required
