Position Details

• Location: Queens, NY
• Employment Type: Full-Time
• Salary: $100,000 annually
• Schedule: Monday to Friday | 8:30 AM – 5:00 PM

Key Responsibilities

• Conduct initial and ongoing assessments of patient health and care needs
• Develop and implement individualized care plans in coordination with physicians and families
• Deliver skilled nursing care, preventive treatments, and specialized services per physician orders
• Monitor patient conditions and adjust care plans as needed
• Document services, observations, and treatment responses in accordance with policy
• Educate and support patients and caregivers on health needs and care procedures
• Supervise and mentor home health aides, ensuring compliance with best practices
• Participate in quality improvement initiatives and performance evaluations
• Maintain patient confidentiality and adhere to infection control protocols

Qualifications

• Education: Associate’s or Bachelor’s Degree in Nursing
• Licensure: Active New York State RN license
• Experience: At least 6 months of clinical nursing experience
• Technical Skills: Strong documentation and care planning abilities
• Soft Skills: Effective communicator, organized, empathetic, and able to work independently or on a team
