Position Overview
We are seeking a compassionate and skilled Registered Nurse (RN) to join a five-star rated Assisted Living Facility in Queens, NY. This full-time role offers the chance to work in a supportive, mission-driven environment where you can make a meaningful impact on the health and quality of life of elderly residents.
Key Responsibilities
- Conduct comprehensive home care assessments and reassessments, using tools such as OASIS when applicable
- Collaborate with physicians, family members, and supervisors to develop individualized Plans of Care
- Deliver skilled nursing services, preventive and rehabilitative treatments as prescribed
- Monitor changes in patient condition and promptly report findings to supervisors or care coordinators
- Maintain accurate documentation of services rendered, condition updates, and response to care
- Provide education and counseling to patients and caregivers on care needs and wellness
- Supervise and evaluate Home Health Aide staff in accordance with facility protocols
- Uphold client rights, confidentiality, and infection control standards
- Participate in performance improvement initiatives and ensure compliance with regulatory requirements

Qualifications
- Education: Associate or Bachelor’s Degree in Nursing
- Licensure: Active New York State Registered Nurse (RN) License
- Experience: Minimum of 6 months of clinical nursing experience
- Technical Skills: Strong documentation, care planning, and assessment capabilities
- Soft Skills: Excellent communication, critical thinking, and problem-solving skills. Team-oriented and independently reliable.
