Physician, Care Management & Palliative Care

Included Health is looking for a compassionate and dedicated Physician generalist who is fellowship trained in Palliative Care to join our Care & Case Management (CCM) Program, where you will play a vital role in supporting members throughout their healthcare journey. Reporting to the Clinical Manager and as part of a multidisciplinary team—including Nurse Care Managers, Dietitians, Pharmacists, Social Workers, and other healthcare professionals—you will provide holistic, patient-centered care through remote consultations and case management. You will develop personalized care plans, offer expert clinical guidance, and empower members to make informed decisions about their health, including managing complex conditions like cancer throughout the care journey. You will collaborate with local treating teams, coordinate resources, and ensure seamless care transitions to improve health outcomes. By analyzing clinical trends and member data, you will also contribute to program improvement and new ideas. • Active medical license and Board Certification in Family Medicine (ABMS or AOA) or Internal Medicine (ABIM), with additional certification or experience in Palliative Care. Candidates without formal certification in Palliative Care but with relevant experience will also be considered.
• 3- year Family Medicine or Internal Medicine residency or equivalent
• 2-years of post - residency experience providing direct care to patients
• Candidates who have 3+ years of post residency experience and can treat all age populations will be prioritized
• Willingness to become and maintain licensure in multiple states to meet program needs.
• IMLC state residency is important (visit IMLC)
• Multiple state licenses are important; Preference to Indiana, Alabama, Georgia, Mississippi, Missouri, Oklahoma, South Carolina, and Tennessee

• Prompt and regular attendance at assigned work location.
• Capability to remain seated in a stationary position for prolonged periods.
• Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment.
• No heavy lifting is expected, though occasional exertion of about 20 lbs of force (e.g., lifting a computer / laptop) may be required.
• Capability to work with leadership, employees, and members in an appropriate manner.

• 8AM-5PM Monday - Friday local time
• Estimated Start Date between March-April 2025 depending on licensing status and notice periods.

The United States new hire base salary range for this full-time position is

$203,330-287.210 +benefits + equity
