Job Summary

Inspire Hospice and Palliative Care continues to grow and is looking for a Hospice Nurse Practitioner (PRN). The Hospice Nurse Practitioners are to: evaluate and assess patients for hospice eligibility at admission or ongoing as needed per Medicare regulation, coordinate evaluation results with the interdisciplinary team, patient/family, referral source, and report results of all evaluations to the Hospice Medical Director. He/she will occasionally provide education to patients, families and other staff members.

Qualifications:

  • Current Graduate of Nurse Practitioner Program.
  • Must possess a valid state driver's license, automobile liability insurance, and have dependable transportation
  • Two years experience as a Nurse Practitioner in a clinical care setting preferred.
  • Previous home health or hospice experience is preferred, but not required.
  • Nursing skills as defined as generally accepted standards of practice. Good interpersonal and communication skills. Specialized skills preferred in palliation and end of life symptom management practice.

Responsibilities:

  • Initial and ongoing comprehensive evaluations of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs and ADLs (i.e. risk for grief, cultural/spiritual, verbal/non-verbal)
  • Evaluate and assess patients to report eligibility determinants to Hospice Medical Director
  • Consulting with and educating the patient/family regarding disease process, self-care techniques, end of life care, nutrition and dietary needs. Providing training to other staff as needed.
  • Documents all assessments and evaluations per agency instruction and policy
  • Informing physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.
  • Evaluating own needs for support using identified systems to meet the need.
  • Communicating information using current process and technology available to the organization.
  • Applying specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility.
  • Proactively completes and documents Face to Face assessments as assigned by the agency, within assigned time frames
