Phamily is assisting in placing a Chronic Care Manager (MA, LPN, or an RN) for Metropolitan Nephrology Associates (MNA), who will implement, manage, and coordinate all aspects of the Chronic Care Management Program activities. Incumbent will directly engage, manage, and triage complex clinical needs while adhering to organizational policies, procedures, and guidelines directed by the patient’s physician or physician assistant.

YOU WILL BE RESPONSIBLE FOR:
- Proactive patient management via text using the Phamily Platform. Triage and respond to patient communication in a timely manner, as per departmental guidelines, for patients requiring medical guidance
- Work closely with in-office providers to manage day-to-day calls involving symptom control, medication management, and patient and family education
- Triage and assess patient health status
- Route patient escalations accurately and expeditiously
- Provide patients with clear instructions and educationally appropriate guidelines (i.e. medication protocol, tests, etc)
- Actively review patient charts to assess health needs and gaps. Follow through with necessary procedures or test requests, pre-approved care algorithms, new appointment times, and referrals to other facilities or services
- Document calls according to established guidelines
- Direct patient to maintain follow-up appointments with providers in-office
- Maintain in-home self-care based on providers' clinical recommendations
- Perform basic and advanced clinical support tasks based on the specialty and under the provider's direction
- Educate the patient on proper outreach for contacting the physician’s office regarding urgent care matters and direct to 911 when appropriate or uncertain
- Communicate and coordinate with physicians, physician assistants, and other members of the health care team to ensure patient compliance with treatment plans
- Work in collaboration with team members to provide outreach and engagement
- Provide a positive patient experience for patients through courteous digital interactions
HOURS: Mon-Fri 8:30am to 4:30pm EST
SALARY: $25.00/ per hour
- Minimum of 2 years experience in Nephrology or Chronic Care Management
- Experience working as a telecommuter or in a call center setting
- Experience in clinical telephone triage
- Experience working with Electronic Health Record (EHR)
- Demonstrates progressive proficiency with the utilization of available computer technology, including typing skills
- Demonstrates customer service, leadership, communication, interpersonal and problem-solving skills
- BLS Certification
- Looking for any/all of the following: Medical assistant (experience and leu of certification will be considered, current LPN or an RN licensure in Maryland and Virginia (graduate from an accredited School of Nursing)
