Advanced Practice Clinician

Our hybrid model allows for both in-clinic and virtual care to bring support to patients where and when they need it.  


As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a virtual primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health. 


Position:  Hybrid 

Schedule: Part time or Full Time Monday-Friday with flexible scheduling (mix of patient contact hours plus administrative time). Primary location: at 330 W Surf St, Chicago, IL.    

EMR: Athena 

On-Call: Phone call only, in a rotation with initial triage by nurses for one (1) week at a time.  Average call volume < 1 call per week.  24/7/365 physician back up.   


Responsibilities:
  • Sees adults for metabolic health and obesity medicine .  This may include sharing a panel with MD/DO + other APCs of the local in-clinic practices. 
  • May see pediatric patients ages 12+ for metabolic health only, based on training, licensure, and competency. 
  • Remote care using audio and visual technology to complete patient examinations, evaluation, assessments, diagnoses and treatment plans.   
  • Embodies the knownwell mission and values of patient inclusivity in how you deliver care. 
  • Coordinates with the broader care team to ensure the patient has the support they need. 
  • Proficiency with Microsoft Office applications recommended, but not required.  
  • Documents as necessary to ensure accurate billing, including review of remote patient monitoring data. 
  • Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response. 
  • Ensure your patient panel is open to any individual, regardless of their current health status and comorbidities. 
  • Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey. 
  • Provide an environment in which patients feel comfortable and free from judgment. 
  • Offer exceptional encouragement as patients reach their goals. 
  • Escalate any concerns to your dedicated site leadership team. 
  • Provide clinical guidance to medical assistants and nursing teams as assigned. 
  • Stay current with licensure and appropriate re-certifications 


Requirements:
  • Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA). 
  • Unrestricted DEA licensure and prescribe authority (per state practice regulation). 
  • At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred.  If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification.   • You have an active, unrestricted license to practice in Illinois, or willingness to obtain.  Willingness to obtain additional licensures if needed based upon business needs. 
  • You have excellent communication and interpersonal skills. 
  • You’re dedicated to staying current with the latest developments in primary care and obesity medicine. 
  • You believe in inclusive, compassionate care. 
  • You believe in shared decision making with a patient. You honor the goals a patient has for their health, and you partner with them to help them achieve those goals.  
  • You have high emotional intelligence and know how to make a patient feel at ease – without judgment and without shame. 
  • You’re willing and excited to try new technologies that are both patient and provider oriented. 
  • You are excited to work with a collaborative team of physicians, APC’s, registered dietitians, medical assistants, health coaches, and other staff. 
  • We are unable to sponsor visas currently, so you are authorized to work in the United States. 
  • Must have access to high-speed internet and phone lines to support a fully remote practice. 
  • You have excellent communication and interpersonal skills.
