Physical Therapist (PT) – Home Health | Mission Home Health

Mission Home Health is seeking a skilled and compassionate Physical Therapist (PT) to join our team in Sun City, AZ. This role offers the opportunity to make a meaningful impact by providing high-quality care to patients in their homes, helping them regain mobility and independence.

Position Details:

Compensation: 

SOC: $115-$120

Revisits: $80-$90

Evals: $90-$100

Hourly Rate/NVA: $35-$40 (education and training)

Employment Type: Full-time

Service Area: Sun City, Surprise, Peoria, Glendale, Phoenix, New River (West of the 51)

Responsibilities:

- Evaluate and assess patients' physical conditions and develop individualized treatment plans

- Implement therapeutic exercises and techniques to improve mobility, strength, and coordination

- Educate patients and caregivers on exercises, safety precautions, and self-care strategies

- Collaborate with an interdisciplinary team to provide holistic, patient-centered care

- Maintain accurate and timely documentation of patient progress and treatment plans

Requirements:

- Active Physical Therapist license in the state of Arizona

- Doctorate or Master’s degree in Physical Therapy

- Minimum of one (1) year of experience as a Physical Therapist (home health experience preferred)

- Current CPR certification

- Reliable transportation with valid driver’s license and auto insurance
