Are you a skilled RN with a sharp eye for detail and a passion for quality care? Join our team as a Quality Care Manager and help ensure that every patient receives exceptional, compliant, and effective care.
Compensation starting at $80,000/year. Open for discussion!
What You’ll Do:
- Review patient documentation and care plans to ensure accuracy and compliance with regulations.
- Confirm medical necessity, skilled need, and alignment with physician orders and homebound status.
- Approve and finalize Plans of Care and OASIS assessments on time.
- Communicate with clinicians to correct documentation issues and support best practices.
- Collaborate with the Quality Director on quality improvement initiatives.
- Use SHP data and reports to improve outcomes and performance.
- Participate in case conferences and help cover weekend/holiday tasks as needed.
- Stay current with home health regulations, CHAP standards, and OASIS guidelines.
What You Bring:
- Active, unencumbered RN license.
- Graduate of an accredited School of Nursing.
- At least 1 year of home health experience.
- Strong working knowledge of OASIS, Medicare Conditions of Participation, and Title XXII.
- Detail-oriented with great communication and problem-solving skills.
- Proficient in EMR systems; Home Care Home Base experience is a plus.
- Ability to work independently and manage time effectively.
