Job Description

The RN/Director of Clinical Services leads our clinical team in providing high-quality care in a home health environment. This leadership role is crucial for ensuring compliance, overseeing medical records, and enhancing patient monitoring processes. The ideal candidate will possess a strong background in case management, supervising staff, and developing care plans that meet the needs of Home Health Patients.

Responsibilities

  • Lead and manage the clinical team to ensure the highest standards of care are maintained.
  • Oversee patient monitoring activities to ensure safety and well-being.
  • Develop, implement, and evaluate individualized care plans for patients.
  • Ensure compliance with all regulatory requirements related to Home Health.
  • Supervise staff, providing guidance and support as needed.
  • Conduct regular assessments of clinical practices and outcomes to improve service delivery.
  • Collaborate with other departments to enhance overall patient care experience.
  • Provide training and mentorship to clinical staff to promote professional development.

Qualifications

  • Proven experience in a leadership role within clinical services in a home health setting.
  • Current up-to-date Registered Nurse License in the state of Florida
  • Strong knowledge of Medicare rules and regulations as they relate to: home health, medical records management, patient monitoring, and case management practices.
  • Exceptional supervisory and employee relations skills.
  • Excellent communication and interpersonal skills to foster a collaborative work environment.
  • Ability to develop comprehensive care plans tailored to individual patients clinical needs.
  • Relevant certifications or licenses in nursing or healthcare administration are preferred.
