• Graduated from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• 1 year of Home Health, Outpatient experience preferred as a Physical Therapist
• Conduct home visit Occupational Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Basic computer literacy skills
• Proficient in EMR documentation (we use Hello Note but experience in this system is not required).
• Interest in working with both Pediatric and Geriatric patients
• Excellent communication between the office staff and therapists
• Maintain up-to-date CPR certification and adhere to all safety protocols
• Manage a minimum caseload of 4-5 patients

• Competitive Rate: $70 - $80 per hour
• Location Flexibility: Select where you work with cases available in all of NY's 5 boroughs and Upstate NY.
