Job Summary

A company is looking for a Bilingual LPN Care Navigator/Certified Care Coordinator.

Key Responsibilities

• Engage in monthly telephonic patient interactions to build relationships and support care goals
• Create individualized care plans and manage coordination via proprietary software
• Document all interactions and escalate clinical concerns as necessary

Required Qualifications

• Licensed Practical Nurse (LPN), Certified Medical Assistant (CMA), or Nationally Certified Pharmacy Technician (CPhT)
• Fluent in Portuguese and English
• Minimum of 2 years' clinical experience
• Experience working with EMR systems
• Intermediate to advanced computer skills
