Care Coach II

Job Summary

A company is looking for a Care Coach II to support family caregivers and their care recipients.

Key Responsibilities

• Engage, motivate, coach, and educate caregivers via phone and digital technology
• Manage a caseload and leverage technology for effective coaching and information referral
• Assess caregiver and care recipient needs and assist in creating care plan goals

Required Qualifications

• 4-6 years' experience in health coaching and/or health care/wellness
• Comprehensive knowledge of industry concepts and principles
• Proven experience accessing and locating community resources
• Solid understanding and experience in care planning/case management
• Multilingual skills are highly desired
