Job Summary

A company is looking for a Telehealth Provider (PA, CNS, NP).

Key Responsibilities

• Conduct telephonic annual wellness visits
• Identify patient needs, provide health education, and offer lifestyle recommendations
• Create and review patient care plans for Chronic Care Management (CCM)

Required Qualifications

• Licensed as a CNS, NP, or PA in Florida
• Willingness to obtain licensure in additional states
• Experience in telehealth, wellness visits, and preventative care
• Compliance with CEU and licensure requirements
• Comfortable using electronic health record (EHR) systems and medical software
