Job Summary

A company is looking for a Social Worker-MSW-Remote.

Key Responsibilities

• Act as a liaison between faith-based partners and the health care team to provide personalized care
• Assess psychosocial risk factors of patients and families and provide counseling regarding illness consequences
• Collaborate with interdisciplinary teams to connect patients and families to community resources

Required Qualifications, Training, and Education

• Master's degree in social work required
• Permanent LSW licensure required; LCSW licensure preferred
• 0-3 years of Medical Social Work experience required
• Knowledge of state and county protective legal mandates and crisis intervention techniques
• Proficiency in Microsoft Excel and Word
