Job Summary

A company is looking for a Social Worker Case Manager, LCSW, Maternity Population (Remote).

Key Responsibilities

• Completes assessments to identify member needs and barriers
• Builds relationships with members and healthcare providers to create individualized care plans
• Coordinates referrals for social services and monitors service delivery

Required Qualifications

• Minimum of 2 years of experience in managed care or a clinical environment with a maternity population
• Valid unrestricted Social Worker license (LCSW) required
• Master's degree in Social Work (MSW) required
• CCM certification preferred upon hire; required within specified timeframe
• Experience with data entry into a database or EHR required
