Job Summary

A company is looking for a Case Manager - RN.

Key Responsibilities

• Receives referrals and conducts comprehensive clinical assessments to determine member needs
• Collaborates with members and families to develop and monitor effective care plans
• Identifies and facilitates access to relevant resources and ensures proper documentation of member data

Required Qualifications, Training, and Education

• Registered Nurse with psychiatric experience or a licensed behavioral health professional
• Minimum of 3 years of relevant direct clinical care experience
• CCM Certification or ability to obtain within 3 years of employment
• Flexibility to work varied hours
• Proficiency in computer applications
