Key Responsibilities

• Conducts clinical assessments and develops comprehensive care plans for members with complex medical conditions
• Collaborates with healthcare teams to ensure effective communication and positive clinical outcomes
• Monitors and updates care plans while identifying cost-saving opportunities and community resources

Required Qualifications, Training, and Education

• Registered nurse with a current MN license and no restrictions
• 5 years of relevant clinical care experience
• CCM Certificate or ability to obtain within 3 years of starting the position
• Experience in managed care is preferred
• Proficiency in computer applications and telephonic communication
