Job Summary

A company is looking for a Telephonic RN Nurse Care Manager.

Key Responsibilities

• Manage a caseload of high-risk patients, coordinating care services and developing personalized care plans
• Facilitate transitions of care to prevent avoidable hospital admissions and ensure patient needs are met
• Provide education on disease management and health maintenance to patients and caregivers

Required Qualifications, Training, and Education

• Active Registered Nurse License
• 2+ years of care management experience in community, health plan, or hospital systems
• Strong clinical skills and proactive thinking
• Knowledge of Medicare regulations and home care standards
• Bachelor of Science in nursing or related field preferred
