Job Summary

A company is looking for a Telephonic RN Nurse Care Manager.

Key Responsibilities

• Coordinates care services with the pod leader to ensure tailored access to comprehensive services for patients
• Develops and manages personalized care plans, addressing care gaps and ensuring access to resources
• Conducts thorough nursing assessments via telephone and provides education to patients and caregivers on health management

Required Qualifications

• Active Registered Nurse License
• 2+ years of care management experience in community, health plan, or hospital systems
• Strong clinical skills and proactive thinking
• Knowledge of Medicare regulations and home care and hospice standards
• Bachelor of Science in nursing or related field preferred
