Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Develop and assess personalized care plans for members, focusing on their physical health needs
• Coordinate care between members, families, and healthcare providers to ensure appropriate services are delivered
• Monitor member progress and update care plans as necessary to meet individual goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Current RN - Registered Nurse license in New York or Compact State Licensure
