Job Summary

A company is looking for an Episodic Care Manager - Medicare Advantage.

Key Responsibilities

• Perform clinical reviews to assess and coordinate health care services for members
• Communicate and collaborate with clinical and non-clinical staff to ensure effective service delivery
• Document outcomes of reviews and analyze clinical information for decision-making

Required Qualifications

• Registered Nurse (RN) with 3 years of clinical experience or Licensed Practical Nurse (LPN) with 5 years of clinical experience
• Valid and applicable clinical license (NC or compact multi-state licensure)
• Experience in Behavioral Health roles may require different licensure with a minimum of 3 years of clinical experience
• Proficiency in Microsoft Office is preferred
• Experience with Medicare and Medicaid is preferred
