• Manage the overall care management of an assigned patient panel
• Establish trusting relationships with patients and families to provide support
• Coordinate interdisciplinary care plans to ensure continuity and reduce fragmentation

Required Qualifications

• Associate degree in nursing
• Current RN License is required, Compact License preferred
• Care management experience required
• Certified Case Manager preferred
• Intermediate skills with MS Office Suite products
