Manage the immediate referral response process to meet performance goals
Conduct clinical assessments based on electronic medical and client relation management systems
Evaluate prospective resident referrals and communicate determinations to referral partners
Required Qualifications, Training, and Education
Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice
Minimum of two years of experience in a similar role within LTC/SNF/AL/MC settings
Experience in clinical liaison and admission coordination
Intermediate to advanced computer skills, including EMR or HER experience
Ability to handle confidential information and make independent decisions
