We’re Hiring: Remote Case Manager/Scheduler (Bilingual – English/Spanish)

Location: Remote (must reside in or near Los Angeles County, CA)
Schedule: Full-Time | Monday–Friday | 9:00 AM–5:00 PM
Pay Range: $23.00 – $26.00/hour (based on experience)
Start Date: Within 2–4 weeks

The Role: Case Manager/Scheduler (Remote)

As a Remote Case Manager/Scheduler, you’ll be at the heart of our care coordination team. You’ll work closely with the Operations Manager and HR to ensure every case is managed with excellence, empathy, and legal integrity. You’ll build strong relationships with caregivers and clients, schedule services that match client needs, and monitor compliance with CDSS and DDS standards.

What You’ll Do
• Provide compassionate, responsive case management for elderly and developmentally disabled clients
• Create and maintain individualized service plans in line with state regulations
• Schedule home care aides in alignment with client preferences, staff availability, and care plans
• Monitor caregiver-client matches and adjust staffing to ensure continuity and satisfaction
• Act as a point of contact for clients, families, and caregivers regarding care logistics
• Collaborate with HR and operations on quality assurance and training follow-ups
• Maintain accurate documentation in compliance with California HCO standards
• Attend (remote or in-person) in-services, mandatory trainings, and emergency meetings
• Participate in ongoing internal and external training opportunities to stay current with legal and industry updates
• Serve as a mandated reporter in compliance with CA law

What You Bring
• Bilingual – Fluent in English and Spanish (Required)
• 25+ years old (Required)
• 2+ years of case management experience or Bachelor’s degree in case management, health/home care, or a related field
• Strong working knowledge of CDSS and DDS systems and requirements
• Stellar communication skills—you’re approachable, professional, and people-savvy
• High personal integrity, time management skills, and accountability
• Passion for person-centered care and supporting vulnerable populations
• Willingness to grow through training and team collaboration
• Must reside in or near Los Angeles County and be willing to attend occasional in-person trainings or meetings
