Job Summary

A company is looking for a Telephonic Care Navigator - Remote in CT.

Key Responsibilities

• Completes telephonic outreach to DSNP/CSNP members and assists with care gaps
• Educates members on healthcare access and connects them to community resources
• Maintains schedule adherence and ensures compliance with HIPAA regulations

Required Qualifications

• High School Diploma / GED or equivalent work experience
• 1+ years of call center and/or telephonic customer service experience
• 1+ years of professional experience in an office setting using telephone and computer
• 1+ years of experience in healthcare, insurance, or community outreach
• Ability to work from home with high-speed internet access
