Background

Since 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities.  The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs.  The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet.  St. Joseph Center serves approximately 6,000 individuals annually.

Brief Description of Principal Activities

Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles.  St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

Job Summary

The Case Manager serves as a team member with several social service professional and paraprofessional staff ensuring the delivery of quality as well as adhering to the mission of St. Joseph Center. The Case Manager will provide case management to homeless families that are residing at SOLA Open Air: A Safe Landing for Families – a transitional housing community for families previously experiencing homelessness in South LA. SOLA Open Air provides housing, as well as supportive services and connection to longer-term affordable housing

ESSENTIAL DUTIES & RESPONSIBILITIES

The Case Manager’s key areas of responsibility include:

 

  • In conjunction with the Program Manager and CES for Families case management staff, evaluate families for permanent housing.
  • Provide case management to families on-site.
  • Be responsible for a caseload of up to 25 families. Establish a case management plan for each family.
  • Provide referrals to any needed services including health, mental health, education, employment, vocational training. Serve as an advocate or liaison to other organizations when needed.
  • In conjunction with the housing specialist, assist families with the identification of appropriate housing and with lease up including negotiation of rental agreement with landlord. On an ongoing basis, act as advocate with landlord as needed.
  • Provide case management to families on-site.

Other duties and responsibilities may be assigned. Qualifications

QUALIFICATIONS

Knowledge, Skills & Abilities:

  • Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people including clients, community, and other staff. Must have excellent administrative and organizational skills.
  • Knowledge of case management principles and intervention techniques specific to homeless populations or other under-served groups.
  • Knowledge of housing market/housing programs is desirable.
  • The position requires an ability to work flexible hours including some early mornings and evenings.
  • Bi-lingual Spanish/English desirable.

Experience:

  • One to two years’ experience providing case management services/advocacy with homeless and low- income families.

Education:

  • Bachelor degree from an accredited college or university in social work, human services, psychology, or a related field. A combination of education and experience may be substituted.

Direct Reports:

  • This position has no direct supervisory responsibilities.

Computer Skills:

  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.

Certificates, Licenses and Registrations:

  • Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Valid automobile liability insurance.

Travel Requirements:

  • Must be able to work onsite at the south la shelter
  •

   Pay Range: $22/hour
