Position Title: Client Care Coordinator
Location: Can work out of any VLRC office across Ontario
Term: Full-time, Permanent
Reports To: Manager, Healthcare Operations – Client Navigation
Salary: $23.12/hour

I. Scope and Summary of Primary Responsibilities
This position includes a wide range of responsibilities, with a primary focus on utilizing the client management system application to facilitate the appointment booking process for specialist and clients within Ontario. You will provide exceptional customer service by effectively communicating, collaborating, and coordinating patient information and bookings. This role involves utilizing the program's electronic medical records software to manage patient information, referrals, and other medical data.
II. Essential Functions
This position entails, but is not limited to the following general responsibilities:
• Completes the appointment booking process for patients within Ontario by utilizing the client management system to assign the necessary services and interventions and schedule appointments through Skedulo.
• Understands and can communicate information regarding VLRC services and resources effectively.
• Identifies and escalates urgent or critical situations to the Manager or other designated personnel.
• Prepares correspondence, manages mailings, and performs filing duties.
• Effectively manages workload and client appointment schedules to ensure efficiency.
• Efficiently processes cancellations and modifications to client appointments and notifies the appropriate staff as necessary.
• Effectively manages workload and client management system caseload.
• Attends both internal and external meetings as required.
• Performs additional duties as assigned.
Relationships
• Effectively collaborates with team members and other departments while also demonstrating the ability to work independently.
Problem Solving
• Operates under the guidance of established processes, adhering to business rules, service standards, and best practices, while ensuring policies and procedures are implemented in a timely manner.
• Engages in service evaluation and program planning as necessary to sustain and enhance service outcomes.
• Contributes to a collaborative team environment to achieve operational and team objectives.
Decision Autonomy
• Operates within the framework of national and divisional guidelines and standards while exercising discretion in making decisions related to routine tasks.
• Consults with the Manager on more complex matters that may require approval.
• Demonstrates sound judgment and professionalism when interacting with individuals and knows when to escalate issues/risks appropriately.
• Dedicated to always maintaining privacy and confidentiality.
III. Qualifications
Knowledge and Skill Requirements
· Possesses a proficient understanding of medical applications and software related to caseload management.
· Demonstrate strong organizational and time management skills.
· Strong sensitivity to the needs of individuals with vision loss.
· Demonstrates effective verbal and written communication abilities.
· Detail-oriented with a proven ability to enter data accurately.
· Proficient in Microsoft Office applications and database management, with a willingness to learn new software applications.
· Familiarity with eye conditions and blindness prevention is considered an asset.
Experience and Education
· A college diploma in Medical Administration and/or a minimum of three years of experience in an administrative role.
· Strong typing and keyboarding skills.
Work Environment
· Ability to work flexible hours.
· Willingness to work in a variety of settings. (hybrid approach)
· Must possess and submit a clear vulnerable sector screen.
· Must be eligible to work in Canada.
