The Role: We are seeking a dedicated and compassionate Family Practice or Internal Medicine Physician to join our innovative team as a Diabetes & Lifestyle/Wellness Specialist. This position focuses on providing high-quality care to patients with diabetes while promoting lifestyle education and wellness. You will have the opportunity to make a meaningful impact on patient health by addressing both clinical management and preventative care strategies. This unique patient population includes retired San Antonio Fire, Police and other First Responders. Travel between all four of our beautiful state-of-the-art Wellness Centers is required; locations include Westover Hills, Fair Oaks, South/Roosevelt and Schertz. Monday – Friday, 8:00am – 5:00pm. No late clinics, no call!

What You’ll Do:
Provide comprehensive care for patients with diabetes, including diagnosis, treatment, and management of comorbidities.
Develop personalized care plans focusing on lifestyle modification, including diet, exercise, and behavioral changes.
Educate patients and families about diabetes self-management, wellness and prevention strategies, and healthy lifestyle habits.
Collaborate with a multidisciplinary team, including dietitians, diabetes educators, Clinical Pharmacists and care coordinators, to ensure optimal patient outcomes.
Utilize evidence-based guidelines to provide medication management and advanced diabetes treatments.
Document patient care and progress in accordance with clinical and organizational standard in patient medical record in the EHR.
Stay up to date with advancements in diabetes management and lifestyle medicine through continued education and training.
Determine which referrals are required based on examination and patient needs.
Determine level of urgency of follow-up, referral/consultation appointments.
Exercise final medical judgment in all issues of health care.
Prescribe medical treatment and clinical drugs to patients.
Order studies, test, and ancillary servicesReview on a regular basis long term cases that require ongoing medical attention.
Consult with Chief Medical Officer, Physician Lead, and other professionals on staff as needed regarding patient care, assessment, and educational issues.
Participate in quality improvement, management, continuing education, and other patient care programs established by Health by DesignAssist as needed in the resolution of complaints, requests, and inquiries from patients.
Assist in the development, production, and presentation of medical education, material, lectures, and mixed media presentations for patient and medical staff.
Maintain confidentiality of all patient information according to federal guidelines and regulations.

Requirements
What You’ll BringEducation and Experience:Education:
Graduate of accredited US School of Medicine; MD or DO
Experience:
Minimum of two years of direct work experience
Certificates, Licenses, Registrations Required:
Current license to practice in state of clinic location (Texas)
Board Certification / Board Eligible (Family or Internal Medicine)
Current DEA Certificate
BLS – Basic Life Support Certification
