CDM Nurse Navigator (RN or LPN) - Palliative Care

  • LPN or RN Palliative Care Nurse Navigator - Comprehensive Disease Management
  • Full Time; Monday through Friday 8am - 5pm
  • St. Louis, MO and surrounding areas

The CDM Nurse Navigator helps bridge the gaps between Home Health and Hospice patients by identifying the needs, recommending resources, communicating with patient's healthcare providers and educating on disease progression and treatment/care options available. Often times people make critical healthcare decisions in crisis and Phoenix's CDM nurses are able to walk this journey with people so they can make informed decisions that align with their personal goals.

Our CDM program is committed to connecting with people who would benefit from assistance in navigating the healthcare system. Phoenix's CDM program is designed to care for people who are living with chronic or life limiting illness and to provide guidance in symptom management and advanced care planning.

Responsibilities

  • Connecting with patients and establishing consistent face to face encounters
  • Managing CDM caseload and making appropriate referrals for homecare and community services
  • Completing accurate and professional documentation after each visit
  • Building and maintaining strong partnerships with community healthcare partners/physicians
  • Connecting and communicating at a high level with Phoenix colleagues to ensure continuity and collaboration of care.
  • Educating on the benefits and goals of palliative and hospice care services

Requirements

  • Must have an active MO or Compact RN or LPN license.
  • Must have active driver's license.
  • Hospice or Home Health experience preferred.
