Job Summary

A company is looking for a Telephonic Care Navigator - Remote in AL.

Key Responsibilities:
  • Completes telephonic outreach to members and assists with scheduling provider appointments
  • Educates members on gaps in care and connects them with community resources
  • Maintains confidentiality of health information and meets quality and performance metrics
Required Qualifications:
  • High School Diploma / GED or equivalent work experience
  • 1+ years of call center and/or telephonic customer service experience
  • 1+ years of professional experience in an office setting using telephone and computer
  • 1+ years of healthcare, insurance, social work, or community outreach experience
  • Ability to attend 100% of the 3-week training period
