Job Summary

A company is looking for a Bilingual ECM Case Manager.

Key Responsibilities

• Coordinate care and serve as the ECM Point of Contact/Lead Care Manager for qualifying youth
• Connect members with essential resources such as housing, food, and childcare
• Collaborate with various healthcare providers to ensure coordinated care

Required Qualifications, Training, and Education

• Master's degree in social science plus one year of experience in behavioral health; or
• Bachelor's degree in social science plus two years of experience in a behavioral health field; or
• Associate's degree in social science plus three years of experience in a behavioral health field
• Valid California Driver's License
• Meets agency driver eligibility criteria
