Minnesota Licensed RN Case Manager

Job Summary

A company is looking for an RN Specialist Complex Case Manager - Commercial.

Key Responsibilities

• Engages with members and providers to conduct clinical assessments and develop comprehensive care plans
• Collaborates with interdisciplinary care teams to monitor and update care plans for optimal health outcomes
• Identifies and facilitates access to relevant resources and programs to support member needs

Required Qualifications

• Registered nurse with a current MN license and no restrictions
• 5 years of relevant clinical care experience
• CCM Certificate or ability to obtain within 3 years of starting the position
• High school diploma (or equivalency) and legal authorization to work in the U.S
• Experience in telephonic outreach and care coordination
