Job Summary

A company is looking for a Senior Care Manager (RN).

Key Responsibilities

• Develops and assesses ongoing care plans/service plans and collaborates with providers to meet member needs
• Coordinates care between members, families, and care provider teams to ensure person-centered services
• Monitors member status and care plan goals, revising plans as necessary in collaboration with members and providers

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 4 - 6 years of related experience
• New York State RN license required
• Pediatric population experience preferred
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
