Develops and maintains positive relationships with members, families, and care teams to enhance health outcomes
Conducts comprehensive health assessments and coordinates care team meetings to implement and evaluate care plans
Monitors service delivery and documentation to ensure compliance with healthcare standards and effective care transitions
Required Qualifications
Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred
Must meet all agency requirements for pre-employment and state regulations
A license or certification in a human service profession, or relevant educational qualifications with experience in the IDD population
Experience in LTSS and/or HCBS coordination is preferred for care managers serving members with LTSS needs
Ability to work autonomously with a high degree of quality
