The Licensed Practical Nurse (LPN) – Long-Term Care/Nursing Home

The Licensed Practical Nurse (LPN) – Long-Term Care/Nursing Home, will provide skilled nursing care to patients in Specialty and Primary Care Clinics in Charleston, SC, and surrounding Community-Based Outpatient Clinics. This role includes managing care for adult psychiatric, medical, and geriatric patients, administering medications, collaborating with the nursing team, and maintaining professional standards in various clinical settings.


$70,000 - $80,000 a year


DUTIES AND RESPONSIBILITIES
·       Deliver nursing care in Acute Care Medical/Surgical areas, outpatient clinics, and labs.
·       Manage care for adult psychiatric, medical, and geriatric patients with increasing independence.
·       Work under the supervision of the Nurse Manager, collaborating with care teams in a variety of clinical settings.
·       Integrate customer service standards into interactions with patients, families, and team members.
·       Administer intravenous medications per facility policies, observing and reporting patient responses to RN or MD/DO.
·       Carry out approved procedures in alignment with the Licensed Practical Nurse Act and VA Qualification Standards.
·       Be available for work in multiple clinical settings, including Acute Care Medical/Surgical, outpatient clinics, and labs.
·       Accurately observe, document, and report patient care activities and progress.
 
MINIMUM QUALIFICATIONS
 
·       Active Licensed Practical Nurse Credentials from ANY U.S. state, District of Columbia, Puerto Rico, or other U.S. territory.
·       Current CPR/Basic Life Support (BLS) certification from the American Heart Association.
·       Degree from a state-approved school of practical/vocational nursing.
·       2 years’ experience: current experience and, (must have worked within the last six months) in a long-term care setting.
·       Must be a United States Citizen and possess the ability to obtain a Public Trust Clearance.
·       Available for 12-hour shifts Monday-Friday, with alternating weekend shifts preferred.
