Internal Care Coordinator

We are a growing behavioral healthcare company looking for a talented copywriter to join our marketing & communications team. In this role, you will be responsible for creating compelling marketing copy for various audiences, including but not limited to patients, healthcare professionals, payors and others. You will write copy for both print and digital assets, as well as help with editing the work of your peers. The ideal candidate will be comfortable in a fast-paced, collaborative environment without sacrificing any attention to detail and can easily work on multiple projects (and brands) at any given time. Compensation Range: $20-24 per hour.

Compensation will be dependent upon geographic region, education, and experience.

The Internal Care Coordinator (ICC) plays a crucial role in collaborating with Clinical Outreach Representatives, Facility Leadership, and Treatment Team members to ensure patients receive appropriate referral options upon discharge. This position serves as a liaison between facility staff and outreach representatives to facilitate seamless aftercare planning and ensure continuity of care.

Duties & Responsibilities

· Act as a key point of contact between facility staff, clinical outreach, and referring providers to support patient aftercare coordination.

· Ensure accurate documentation and tracking of aftercare plans within EMR and CRM systems.

· Support treatment teams in identifying and coordinating appropriate aftercare referrals.

· Participate in facility meetings and provide training sessions to new team members.

· Collaborate with internal departments to address patient needs and improve processes.

· Maintain confidentiality and compliance with all organizational policies and procedures.

· Strong data management and documentation skills.

· Ability to multi-task and prioritize effectively in a fast-paced environment.
