You will write copy for both print and digital assets, as well as help with editing the work of your peers. Compensation Range: $20-24 per hour.
Compensation will be dependent upon geographic region, education, and experience.

The Internal Care Coordinator (ICC) plays a crucial role in collaborating with Clinical Outreach Representatives, Facility Leadership, and Treatment Team members to ensure patients receive appropriate referral options upon discharge. This position serves as a liaison between facility staff and outreach representatives to facilitate seamless aftercare planning and ensure continuity of care.
Duties & Responsibilities
· Act as a key point of contact between facility staff, clinical outreach, and referring providers to support patient aftercare coordination.
· Ensure accurate documentation and tracking of aftercare plans within EMR and CRM systems.
· Support treatment teams in identifying and coordinating appropriate aftercare referrals.
· Participate in facility meetings and provide training sessions to new team members.
· Collaborate with internal departments to address patient needs and improve processes.
· Maintain confidentiality and compliance with all organizational policies and procedures.

· Strong data management and documentation skills.
· Ability to multi-task and prioritize effectively in a fast-paced environment.
