Summary

The Women’s Center Case Manager II supports women in immediate need of housing, basic items, food, health services, and other resources due to homelessness, gender-based violence, and trauma. The Case Manager II provides counseling to women seeking recovery from substance use issues, engaging in activities that promote stabilization and well-being.

This role involves assessing applicants, developing service plans with SMART goals, and maintaining thorough documentation, including assessments, service plans, and progress notes. The Case Manager II facilitates recovery groups, workshops, and psychoeducation sessions to enhance clients’ self-sufficiency and growth mindset.

Work is primarily onsite and in the field, including community engagement. Regular hours are Monday through Friday, 9:30 a.m. to 6:00 p.m., with occasional evening, weekend, or early morning work as needed.

• Manage a caseload of at least 15 clients with a trauma-informed, client-centered approach.
• Conduct client assessments, develop service plans, maintain progress notes, and ensure timely follow-up.
• Identify participant strengths and barriers, connecting them to appropriate resources.
• Facilitate daily/weekly workshops and groups that support recovery and personal growth.
• Navigate social services to ensure clients have access to needed resources and community connections.
• Maintain professional boundaries and adhere to ethical practices, including confidentiality and regulatory compliance.
• Attend staff meetings, community events, and professional development training (minimum of 15 hours annually).

• Minimum of 3 years’ experience in recovery-oriented services for high-risk women or a Bachelor’s degree in social work/psychology with 2+ years of related experience.
• Strong case management and group facilitation skills.
• Familiarity with the Black/African American community’s specific challenges is highly preferred.
• Proficiency in assessment, intervention strategies, and harm reduction practices.
• Excellent communication and conflict resolution skills.
• Strong attention to detail for documentation and database management.
• Familiarity with San Francisco resources and benefits programs is helpful.
• Experience in community outreach is preferred.
• Knowledge of Microsoft Word, Outlook, and Excel required.
• Familiarity with ONE System and Salesforce strongly preferred.

• Required: Duluth Facilitator Certification and Domestic Violence Certification (can be obtained after hire).
• Preferred: Peer Specialist or SUD Certification.
